Jackie S. Kazmark, of Pioneer Lane, Delevan, died Monday January 9, 2017 in Bertrand Chaffee Hosptial, Springville at the age of 55. She was born July 17, 1961 in Springville a daughter of Michael Kazmark, Sr. and the late Phyllis A. (nee Hatch) Renik. She was a homemaker and a member of St. Mary's Church, East Arcade and the NRA. She is survived by her father - Michael Kazmark, Sr. of East Otto; two brothers - Michael, Jr. (Lisa) Kazmark of Centerville and Terry (Heidi) Kazmark of East Otto; four nieces - Crystal, Amber, Jessica, Michele; a great-niece - Ava; step-siblings - Polly Renik, Trisha Renik, Ella Booth, John Booth, Lisa Pool, Dave Booth, Linda Thompson, Billy Booth and her companion - Leo Cummins of Machias. She was preceded in death by her mother and step-father - Phyllis and Daniel Renik; step-mother - Jeanette Kazmark and a step-brother - Chuck Booth. There are no calling hours. A memorial service will be held Thursday January 12 at 10 am in W. S. Davis Funeral Home, Inc., 358 W. Main St., Arcade with Rev. Joseph Gullo officiating. Burial will be in Maplewood Cemetery, Springville.
